Instructions
You are logging in to the PT Board web site. Logging in does not necessarily get you access to all functions. When you have logged in, you will return to the page you were on.
Licensees:
Your User ID is your license number preceded by P for PTs and A for PTAs. (Betty Smith, PT = P98989 or John Smith, PTA = A89898).
Applicants:
You User ID is your your applicant ID, which was printed on your applicant letter. If you do not know your password, follow the instructions
below to reset your password.
If you do not know your User ID:
Select "Retrieve User ID" from the menu on the left. You must enter the email address you have on file with the NC Physical Therapy
Board to retrieve your ID. Your email address must be valid and match the one file with the board.
Password / Security Code
Your original Password /Security Code has been printed on your license renewal card information for the past
several years. Your original Password / Security Code was the last four letters of your first name and the last
four numbers of your Social Security Number. For example: Betty B. Smith, SSN# 555-66-1234 – etty1234. If
your first name only has three letters, use last four letters your last name. If you have changed your
Password / Security Code, you can request a password reset via the menu at the top left hand corner. You can
also change your Password / Security Code using the top left hand menu.
